Icatibant is a selective bradykinin B2 receptor antagonist and a synthetic decapeptide used for the treatment of acute attacks of hereditary angioedema (HAE). As an orphan drug, icatibant competitively blocks bradykinin binding to B2 receptors, inhibiting bradykinin-mediated vascular permeability increase and edema formation, thereby rapidly relieving acute symptoms in HAE patients. HAE is caused by deficiency or dysfunction of C1 esterase inhibitor (C1-INH), with bradykinin metabolism imbalance as the core mechanism of edema. Icatibant, as a B2 receptor antagonist, directly blocks bradykinin signal transduction, terminating the pathological cascade at the downstream level. Compared with C1-INH replacement therapy (requiring intravenous infusion), icatibant offers more convenient subcutaneous administration; compared with androgen prophylaxis, icatibant avoids hormone-related adverse effects with superior safety profile. |
