Liraglutide is a 31-amino acid synthetic peptide analog of human glucagon-like peptide-1 (GLP-1) with 97% amino acid sequence homology to endogenous human GLP-1(7-37). It is a long-acting GLP-1 receptor agonist featuring a unique C16 fatty acid (palmitic acid) side chain conjugated to Lys26 via a glutamic acid spacer, enabling prolonged half-life through albumin binding. Liraglutide is the first acylated GLP-1 analog to achieve once-daily dosing, establishing the foundational design principle for modern GLP-1 therapeutics. Liraglutide binds to and activates the GLP-1 receptor, a G-protein coupled receptor expressed in pancreatic islet beta-cells, hypothalamus, and other tissues. Receptor activation increases intracellular cyclic AMP (cAMP), triggering glucose-dependent insulin secretion from beta-cells while suppressing glucagon secretion from alpha-cells at elevated glucose concentrations.
